Section Headers are a way of labeling and describing related sets of questions in your form. You can add them as you create questions, or add them later.

To add a section header to your form

1. Go to your Form Summary page and click the Questions tab
2. Click Create a Section Header
3. Fill in the Header Title and (optional) Additional Description
4. Click Save
